Every adult needs a healthcare directive. This is a power of attorney that allows another person to make medical decisions for you when you are unable to make those decisions yourself. You have probably heard of Physician Directives and Living Wills. These are now combined in what attorneys refer to as a Health Care Power of Attorney ("HPOA"). The HPOA is limited to healthcare and end-of-life decisions.
In the past, we have charged our clients to create this important document, however, we believe the HPOA is so important that we are offering this to the general public as a community service. Many of the forms being used by hospitals and at "legal form shops" are entirely inadequate and incomplete. Though the language of this document is copyright protected, you may use your form without limitation for your individual use.
Below, we are asking you to fill out basic information that we need to create your form. We will retain a copy of your form in our files.
